Carson City Health and Human Services 5-Year Strategic Plan
This Strategic Plan was developed in the latter half of 2024 and is intended to guide the direction of services for the agency from 2025 through 2029.
Community Health Improvement Plan
The Quad-County Community Health Improvement Plan (CHIP) was developed in response to the Community Health Needs Assessment. The CHIP was completed with input from community-based organizations and community members across the Quad-County Region, including Carson City, Douglas, Lyon, and Storey counties. During a day-long planning meeting, 23 different organizations participated in developing action plans for each priority area identified in the Community Health Needs Assessment. The goal of the CHIP process is to measure and assess the effectiveness of key strategies identified in the action plan. This CHIP will run through 2029.

Community Health Needs Assessment
This Quad-County Community Health Needs Assessment (CHNA) serves as a critical phase in the overall effort to improve community health. Through the collaborative CHNA process, we create a snapshot of community health perceptions, health behaviors, and disparities. We develop an inventory of community assets and resources available to catalyze progress, and we reach consensus on our top community health priorities.
